Vaginal Atrophy in Trans Masculine Folks

Introduction

The vaginal epithelium (vaginal skin cells) depends on estrogen for overall function, especially lubrication. In a person assigned female at birth, not using masculinizing gender affirming hormone therapy, the body will signal the brain to release chemicals to stimulate the production of estrogen. Estrogen travels via the blood stream to the cells of the cervix and vagina resulting in different types of genital discharge. Testosterone in the blood signals to the brain to stop the hormones that trigger the production of estrogen. While increased levels of testosterone in the blood leads to masculinization, it can cause atrophy of the vaginal cells.

Symptoms

Vaginal atrophy may be experienced differently in each individual. It may feel like burning when you pee, going more often to urinate, painful penetration, and overall genital discomfort.


Treatment

There is topical estradiol cream that health care providers can prescribe. In my own health care practice, I advise starting with applying it three times per week.


Discussion

Topical estradiol cream will not affect your masculinization. The cream is absorbed into local tissues. If it does enter the blood stream, the levels of estrogen are not high enough to counteract masculinization. 

I understand that not being able to bottom for front hole sex may cause concerns. That is perfectly valid. 

If you are comfortable, try using the estradiol cream. 

Will estradiol cream affect bottom growth?

No, it should not affect bottom growth. Bottom growth is related to the levels of testosterone in your blood. When applied topically, the Estradiol is not likely making it into your blood stream. If it does get absorbed by the vaginal cells into the blood, the concentration is not high enough to affect masculinization.

Bottoming

You are a valid bottom no matter your ability to have penetrative sex. 

I invite the use of the two other holes, if that feels good for you. 

You can bottom for non penetrative sex like bukkakes and for kink activities like bondage, impact, and more.
